STRIKE BOXING - MEMBERSHIP POLICY AGREEMENT

This Membership Policy Agreement (“Agreement”) outlines the terms and conditions of membership at Strike Boxing & Fitness Club, LLC (“Strike Boxing,” “we,” or “the Club”). By purchasing a membership, the Member agrees to the policies below.
1. Membership Options & Pricing
12-Month Unlimited Membership Contract: $130/month (auto-billed monthly)
Month-to-Month Unlimited Membership Contract: $145/month (auto-billed monthly until cancelled)
Annual Unlimited (Paid in Full): $1,200 (non-refundable except as outlined below)
Youth Membership (ages 5–13): $99/month (youth classes only) (auto billed monthly)
Pricing is fixed for the duration of the stated term and may increase upon renewal.
Membership Includes: Adult memberships include all adult classes, access to the workout area, open bag time, virtual boxing classes Youth memberships include youth classes only.
No enrollment fees.
2. Billing & Payments
Accepted payment methods: Credit/debit card or Venmo.
Monthly memberships will be billed each month automatically on the anniversary date of signup. All annual contracts will renew as a “Month-to-month” membership with the associated pricing adjustment unless a new annual contract is created.
Annual paid-in-full memberships are charged in full at the time of purchase.
Strike Boxing charges no late fees.
If a card declines, the membership will immediately be placed on pause for up to 60 days. If payment is not resolved during that window, the membership will be cancelled, and 60 days of membership dues will be billed.
3. Freeze Policy
Members may freeze their membership under the following terms:
Up to 30 days total per calendar year
No documentation required, simply let management know that you need to pause your membership temporarily, and for how long.
No freeze fee
Freeze requests must be submitted in writing (email, text, or in person)
Frozen time does not count toward the 12-month contract term.
4. Cancellation Policy
A. Standard Cancellation (Early or End-of-Term)
Strike Boxing allows early cancellation with the following terms:
Billing continues during the 60-day notice period.
B. Utah-Required Cancellation Rights (Health Spa Services Act §13-23)
Members may cancel at any time for the following reasons without penalty:
Relocation more than 25 miles from the gym
Death or disability that prevents use of the facility
If Strike Boxing stops offering services included in the membership
Strike Boxing will require proof for these cancellations.

Members may book or cancel classes anytime through our booking app. This policy may change at any time.
No late-cancel or no-show fees. This policy may change at any time.
Strike Boxing reserves the right to adjust the schedule, class caps, or instructor assignments as needed.
7. Safety, Conduct, & Liability
All members must sign and comply with the Strike Boxing Liability Waiver.
Members must follow all gym safety rules, staff instructions, and posted signage.
Unsafe, threatening, or disruptive behavior may result in suspension or cancellation of membership.
8. Children & Minors
Minimum age for youth membership: 5 years old
All youth memberships are limited to youth classes unless otherwise approved.
Anyone under 16 must be accompanied by a parent/guardian at all times while in the facility.
Parents are responsible for the behavior and safety of their children inside the facility.
Anyone under 16 is not allowed in the workout area or any gym equipment.
9. Dress Code
To ensure member safety and protect equipment:
Closed-toe, non-marking athletic shoes are required at all times (except briefly when changing footwear or using the shower).
Members must use wraps/gloves in accordance with class requirements.
10. Equipment Use Guidelines
Read instructions on all equipment before use.
Ask staff for help if unsure how to use equipment.
Use proper form and do not overexert yourself.
Always use a spotter for heavy lifts.
Consult a doctor before beginning any new exercise routine or diet.
Report any damaged equipment to staff immediately.
Members use equipment at their own risk, per the Liability Waiver.
11. Privacy Policy
Strike Boxing collects personal information (name, contact info, billing details) for membership management and communication.
We may process usage data such as class attendance to improve programming.
You may receive transactional and optional promotional emails/texts.
You may opt out of promotional communication at any time.
Strike Boxing will never sell or share your personal information.
Information may be disclosed if required by law or to protect member safety.
12. Photography & Video Policy
To protect member privacy:
No photography or video in restrooms or other private areas.
Do not record or post images of other members without their express consent.
Respect privacy and do not share identifiable information about members or staff online.
Strike Boxing may occasionally take photos/videos of classes for marketing; members may opt out by notifying management in writing (text or email).
13. Utah Statutory Health Spa Required Disclosures
(Required by law; included verbatim in compliant form.)
Strike Boxing & Fitness Club, LLC is registered with the Utah Division of Consumer Protection. Members have the following rights under Utah Code §13-23:
Contracts may not exceed 36 months.
Members may cancel without penalty if the facility goes out of business, relocates more than 10 miles away, or stops offering services.
Future services (prepaid memberships) cannot exceed 12 months beyond the date of purchase.
Any prepaid amounts for unused services will be refunded within 30 days of cancellation under statutory conditions.
